Who We Are
This Notice applies to services provided by Jessica Wimer, RN-IBCLC, a licensed Registered Nurse and International Board Certified Lactation Consultant operating under Born and Fed, a private practice based in Pennsylvania. We are committed to maintaining the privacy and security of your health information.
Our Responsibilities
We are required by law to:
Maintain the privacy and security of your protected health information (PHI).
Provide you with this notice of our legal duties and privacy practices.
Follow the terms of this Notice currently in effect.
How We May Use and Disclose Your Health Information
We may use or share your health information in the following ways:
For Treatment
To provide, coordinate, or manage your lactation care. This may include communication with your or your child’s other healthcare providers (e.g., OB, midwife, pediatrician).
For Payment
To obtain reimbursement for services provided. This may include sharing necessary information with your health insurance provider or billing service.
For Healthcare Operations
To support internal operations such as quality assessment, training, customer service, and administrative processes.
For Telehealth Services
We offer consultations via telehealth, which may include video, audio, and/or secure digital communication. Your PHI will be protected in accordance with HIPAA regulations during all virtual sessions. We use secure, HIPAA-compliant platforms, but you should be aware there is always some risk when communicating over the internet.
Other Uses and Disclosures Permitted or Required by Law
We may also disclose your information under the following circumstances:- Public Health and Safety – For reporting births, communicable diseases, or to prevent serious threats.
Child Abuse and Neglect – As a mandated reporter in Pennsylvania, we are required by state law to report suspected child abuse or neglect to ChildLine and the Pennsylvania Department of Human Services.
Health Oversight Activities – For audits, investigations, inspections, and licensure by government agencies.
Judicial and Administrative Proceedings – If ordered by a court or administrative tribunal.
Law Enforcement – To comply with laws and valid legal requests.
Research – With your written authorization or if the research has been approved by an Institutional Review Board (IRB) with adequate protections in place.
Business Associates – We may share your information with third parties who perform services on our behalf (e.g., billing, IT), under HIPAA-compliant contracts.
Your Rights Regarding Your Health Information
You have the right to:
Inspect and obtain a copy of your health records, including those used to make decisions about your care.
Request an amendment if you believe the information is incorrect or incomplete.
Request confidential communications (e.g., via phone, email, or specific address).
Request restrictions on how your health information is used or disclosed.
Receive a list (accounting) of certain disclosures made of your health information in the past six years.
Receive a copy of this notice in paper or electronic format.
Revoke your authorization at any time, in writing, for future uses and disclosures not required by law.
Pennsylvania-Specific Rights and Notices
Under Pennsylvania law, additional protections may apply to certain health records such as mental health, HIV status, and drug/alcohol treatment records. These are only disclosed with your explicit written consent, except as required by law.
As a licensed professional in Pennsylvania, I am a mandated reporter and must report suspected abuse or neglect to state authorities.
